1. In case of emergency, I hereby give permission to the physician selected by the camp director to secure proper treatment for my child as named on this form. I understand every effort will be made to notify the camper's parents or guardian in such circumstances. I also allow my child's attendance under the terms and conditions of this camp.
2. In signing this application, I, the parent or guardian, certify that the applicant is normal in condition and habits and is amendable to necessary discipline. I accept that the director reserves the right to dismiss a camper who, in his opinion, is a hazard to the safety and the rights of others, or who appears to have rejected the reasonable controls of camp.
(NO refund for early dismissal)
3. The signature of the parent or guardian on this application shall give Bethel Gospel Camp the right to use photos or video of the camper for promotional purposes. (Names of campers will not be disclosed and will kept confidential.)
*Do NOT bring electronics such as music players, video or game players, iPods, cell phones, etc.
*Please no junk food
All children’s camps (includes Teen camps) have daily snack shack treats, except Teddy Bear.

If cancelling: please give 3 weeks notice to receive full refund. (does not apply to camps which ask for a donation for the fee).
To reserve a spot at camp: payment must be received. If paying off-line (cheque, cash, e-transfer–when available), please provide (print) the completed registration form(s) along with the appropriate funds so we can reserve your spot at one of our great summer camps.

2. In the notes of E-Transfer please put your name, the name of camper, and the week you are registering for.
